Please complete this form in order for us to best match you with a dog.

Submitting an application does not commit you to adopting the dog, nor does it guarantee an adoption will result from it’s submission. Your application allows us to start the assessment as to whether the dog and potential adopter are a good fit.

Applications are reviewed in the order in which they are received. If your application is selected, one of our Placement Team members will give you a call to tell you more about the dog, answer any questions you may have and to find out more about your household.

If we mutually feel it may be a fit, and after your references are verified, a meet and greet will be scheduled.

IF YOU ARE LIVING IN A RENTAL PROPERTY, WRITTEN APPROVAL FROM YOUR LANDLORD OR A COPY OF THE LEASE SHOWING THAT YOU ARE ALLOWED TO HAVE A DOG ON/IN THE PROPERTY IS REQUIRED PRIOR TO THE MEET AND GREET.

ALL members of the household, INCLUDING canine members need to be present at the meet and greet.

PROOF OF UP TO DATE VACCINATIONS FOR CANINE FAMILY MEMBERS IS REQUIRED PRIOR TO THE MEET AND GREET.

If the meet and greet goes well, and we mutually feel it is a match you will be able to take the dog home the same day.

Two personal references are needed.  PLEASE NOTE that FAMILY MEMBERS & SIGNIFICANT OTHERS WILL NOT BE ACCEPTED AS A REFERENCE.  References will be checked prior to scheduling a Meet & Greet.

Factors such as number of hours alone, methods of confinement, yard/fence size, puppy/dog breed experience, number of current dogs in the household, and needs of the particular dog - exercise, medical and behavioral - are all considered for placement.

This application has 5 sections: About You, About Your Household, Other Pet Information, About Your Lifestyle and Personal References.

Please note that this adoption questionnaire will be kept on file for 90 days. If the dog you apply for is adopted by another family, you may apply your application to another adoptable dog on our website within 90 days of it’s submission by simply emailing the name of the dog you would like your application applied to at: adopt.hraad@gmail.com.  If you choose to adopt after 90 days  you may be asked to complete another questionnaire.
