In order to request time off- Fill out this form, and all clinical coordinators and BCBAs will be alerted. You will receive an email from your clinical coordinator or BCBA approving the time. Once you've received this approval, you will enter the time into Aloha using the pay code "Paid time off" and in the notes section include who approved and the date. This entry is very important as this is how the time will get entered into Heartland for payment. If you do not receive email approval within 1 week of the request, please reach out to information@littleoaksaba.com so that we can appropriately record and approve the request.
As a friendly reminder, we do ask for at least 2 weeks notice for any planned absences so that we can schedule staff accordingly to protect each child's service schedule. When you receive that email approval, you'll be advised on whether you need to notify the family.
PTO accruals are reserved for full time staff only. Full time status is defined as a minimum average of 32 hours/week and begins after your first 90 days of employment.
