MQH Basketball Leagues Guest Player Declaration Form

All guest players must be registered and approved on a weekly basis before they are allowed to participate in league games. Please confirm your team roster with the scorebook before each game. No team may have more than two (2) guest players participate in the same game unless otherwise approved by the MQH Athletic Board. Guest players participating on your team should be of a similar ability level of the roster player that is absent that weekend. Guest players will not be permitted to participate in league tournament play, but may permitted to participate with an older team/division during tournament play at the discretion of the MQH Athletic Department. All requests must be made to the MQH League Director by Thursday 5:00pm of the current weeks game(s). A listing of approved guest players will be provided to the scorekeeper each weekend to be manually added to the scorebook of your game. Failure to abide by these guidelines will result in a forfeit of your game for the upcoming weekend.
