This form is for all students renting a school instrument for the 2020-21 school year. If you are renting more than one instrument, you will need to fill out a form for each instrument. Yey, paperwork.

RIGHTS AND OBLIGATIONS: This contract grants the student the right:1. To use the instrument for the duration of the contract period, as outlined below 2. To receive the instrument in clean and proper working order.This contract obligates me:1. To be responsible for normal wear-and-tear repairs. Repairs will be made by an authorized instrument repair shop, approved by the Band Director. 2. To be responsible for any damage to this instrument either by paying for repair by an authorized instrument repair shop or by providing for a comparable replacement of the loaned instrument,a. In instances when the instrument is being used by more than one individual, the financial responsibility for necessary repairs/replacements shall be shared between the parties involved, 3. To actively participate in and regularly attend instrumental music classes and performances, 4. To provide evidence of practice through continued improvement, 5. To ensure that no unauthorized individual has access to this instrument, 6. To maintain all necessary cleaning, care, and maintenance. 7. To provide my own supply of consumable accessories (reeds, valve oil, cleaning supplies, etc.), 8. Upon request, to return the instrument to the school in satisfactory working condition.CONDITION: This instrument has been inspected and I acknowledge that, at the time of check-out, the instrument was in good operating order *

INSTRUMENT USE PERIOD: This instrument use agreement is for the time period beginning (see below) and ending June 14, 2021. If a student stops participating in the school’s music program, the instrument must be returned promptly. The district reserves the right to modify the return date of the instrument at the district’s discretion. Failure to return the instrument by the specified date will result in additional fees.

STANDARD SCHOOL-YEAR RATE: $150.00 for the school year (this includes summer and initial cleaning)

STANDARD SCHOOL-YEAR RATE: $25.00 for just the summer

SECOND INSTRUMENT RATE: $100
