1.所有已通過[動物中心使用說明會]者皆可申請門禁權限

2.需使用(含)2020年後的[動物中心使用說明會]出席證明申請權限

3.單次開通以2年為限

4.2年後需再次參與[動物中心使用說明會]並通過考試，通過後請聯絡承辦人做延續開通作業

5.可拿欲開通權限之感應晶片卡感應門禁機，如門禁機有反應，即該卡可設定開通權限

6.如需開通特殊門禁(3F行為學儀器室)，請通過行為學儀器操作考試後聯絡門禁承辦人開通權限

注意:每月月底表單將會關閉，日期更新完成後再開啟

2. It is necessary to pass the test of [Introductory seminar of LAC] which was hold after(include) 2020

3. After 2 years, you need to participate in the seminar again and pass the test. After passing the test, please contact the undertaker to continue the opening operation.

Note: The form will be closed at the end of each month and will be reopened after the date update is completed
