Please consider the following when making your decision.  It is important to note that there is a high likelihood that K-5 students will return in-person 5 days per week at some time during the second semester.

1. This form closes on Tuesday, November 24,  2020, at 11:59 pm. There will not be any additional opportunities to change your student's learning plan beyond the date listed above.
2. All changes to a student(s) learning plan changes will go into effect after winter break on January 4, 2021.
3. Families are committing to their decision for the remainder of the 2020-2021 school year.  Once TVLA is selected, a student cannot return to in-person instruction.
4. Regardless of any change to the public health threat level once TVLA has begun, all instruction in TVLA will remain online.
5. Students transitioning to TVLA will have a different teacher(s) than their currently assigned teacher(s).
6. Students transitioning to TVLA who have an IEP will have a different Intervention Specialist than the one currently assigned.
7. Students returning to in-person instruction may not be placed back with their previous teacher(s) or team.  
8. Grades PreK-5 will include synchronous and asynchronous instruction.
9. Daily attendance will be taken and students are expected to attend daily live instruction sessions.
10. TVLA students who receive special services (special education, related services, 504 accommodations, or English as a Second Language supports) should have conversations with their educational team prior to making their decision to determine how the virtual learning model will be effective in meeting each student’s unique learning needs.

1. If your student(s) is/are currently participating in TVLA and you wish to continue participating in TVLA for the remainder of the 2020-2021 school year, there is nothing additional you will need to do. You do not need to submit this form.

2. If your student(s) is/are currently participating in TVLA and you would like for them to return to hybrid/in-person instruction for the remainder of the 2020-2021 school year, please complete and submit the form below.

3. If your student(s) is currently participating in hybrid/in-person instruction and you would like to enroll them in TVLA for the remainder of the 2020-2021 school year, please complete and submit the form below.
