Complete this form by May 14th if you are interested in pursuing middle school leadership. After applying keep a close eye on your school email as each course requires try-outs or interviews with specific dates/times & each has limited spots available.

Our leadership programs have GPA requirements (no D's, F's on report cards or progress reports). Do you currently have C (70%) or better on your progress report and report cards? *
If you do have less than 70% in a category, please explain the area of needed improvement.
Our leadership programs have Social/Behavioral requirements (no discipline points or referrals). In the past year,  if you have received disciplinary action, please explain.
