I can confirm that as the student named, I have read and fully understand the Elite Team contract linked above.  I understand that should my application be successful, I will have made a 12-month commitment, running September - August, that includes attending all classes, performances, and rehearsals.  I have read this contract with my parents/guardians and have discussed the responsibility I am willing to undertake.  I understand everything stated in this contract and am excited to agree to be a conscientious and reliable member of the Elite Teams for the upcoming season should my application be successful. *
I can confirm that as the parent/guardian named, I have read and fully understand the Elite Team contract linked above.  I confirm I have discussed the information above with my child.  I understand that any fees due must be paid in a timely fashion.   I also understand that my child’s participation is vital to the instructor and other members of the team and therefore I will not revoke my child’s participation in the team as a punishment or because of other activities/commitments we may make at a later date, and that doing so may jeopardise my child’s current and future status in the team.I understand that the main form of communication is via the closed Facebook group and I will check this on a regular basis.  I have read this contract with my child and have discussed the responsibilities we are willing to undertake.  I understand everything stated in this contract and am excited to be a supportive and helpful parent member of the Elite Teams should this application be successful. *
