This is a pre-order reservation for EARLY BIRD by Bonnie and Camille for Moda. You will receive an invoice for the items you reserve. You are expected to purchase the items chosen on this form. Any cancellations or changes must be made within 48 hours of submitting this form.

All prices listed do NOT include shipping. The least expensive shipping will be chosen to ship your order. If there is a postal increase before your order is shipped, you will be responsible to pay the increase before your order will be shipped.
