Dear Participant,

You have been asked to participate in the study described as follows. Should you have any questions regarding this study, feel free to direct them to me or research supervisor. Our details are in this consent form. To participate in this study, you must be between 18 to 28 years old, who has been bullied and currently self-harms.

You are invited to participate as a subject in the research project titled “The Mediating Effects of Self-Stigma and Depression on Bullying and Self-harm among Young Adults”. The aim of this project is to investigate the correlation nature of bullying and self-harm which can be mediated by depression and self-stigma.

Your involvement in this project will be online where you will be asked to fill up five scales that includes demographic questionnaire, Bullying Scale for Adults (BSA), Internalized Stigma of Mental Illness Scale (ISMI), Patient Health Questionnaire (PHQ-9) and Self-Harm Inventory (SHI). It will take no longer than 20 minutes to complete the whole study. If at any time for any reason, you wish to terminate your participation in this study, you may opt out of the google form link. As it is your choice to participate in this study, you may choose to terminate your participation as you see fit. Thus, there will be no action taken against your termination.

The project is being carried out as a requirement for Masters of Clinical Psychology by Inderpreet Kaur under the supervision of Ms. Faeza Hasnan who can be contacted at:
