DROP OFF: 5:30am Petsmart, Davis Highway (by the ihop sign)
PICK UP: Same Location, will text when returning, usually pickup around 6pm SAME DAY
Cost: $80 includes a spay/neuter, Rabies vaccine and FVRCP Vaccine.

FVRCP w/FeLV: +$8 (base FVRCP is included, this is the additional w/ FeLV)
Revolt: $10-15

Cats must be in hard carriers with a blanket/towel/or pee pad.

Carriers must be marked correctly: Your name, your phone number, cats name, cat's sex, Spay/Neuter, FVRCP, Rabies, EAR TIP YES or NO

Ear Tip? This is recommended for outside and/or community cats. *
I acknowledge that my cat(s) must be in a hard carrier no larger than 24'' x 14'' with a blanket/towel/or pee pad. This is a clinic requirement.

I acknowledge that I must pay a $25 deposit upon receiving confirmation that I have a spot. Full payment due 72 hours prior to trip.

I would like to donate additional funds to a general spay/neuter fund for community cats to be used at PFK's discretion
 *
I acknowledge that I will be responsible for paying PFK any potential incidentals that may occur during surgery as deemed medically necessary by the veterinarian. I will pay PFK promptly upon request and will follow up directly with the clinic if additional questions surrounding the charges arise.

For example: 
$15 subcutaneous fluids for a pregnant cat
$30 antibiotic shot for cats with wounds OR cats who may be more prone to infection following a difficult surgery
