Group Form Interest Questionnaire
This form is to gauge your interest in potential group guitar or ukulele classes to be offered by the Milwaukie School of Guitar.

Your feedback will help determine the types of courses to be offered and other critical details. You will also have the opportunity to be notified first when registration opens for future classes.

Most courses will be taught in a series of 4-5 session, but standalone courses are possible as well. All lesson materials will be provided except for instruments.

Please note: Proof of Covid vaccination or weekly negative Covid-19 test result will be required.

Class location will be determined by number of students, but classes will likely take place at the Milwaukie School of Guitar or the Chapel Theatre (near King Rd. Safeway).
