- your child has developed symptoms between Monday 29th March and Friday 2nd April and has tested positive OR
- your child was asymptomatic and has tested positive before Tuesday 6th April

If symptoms started after Friday 2nd April (ie more than 48 hours after last being in school) you do NOT need to complete this form.  

If your child is unable to return to their bubble when school starts again on Monday 19th April, please notify the school in the usual way.
