School Choice Scholarship Form

Due to the legislative changes and to better serve our students, we are updating many of our processes for scholarship students. 

If you are trying to approve your student's course request: Please make sure you exit this browser and click the blue 'approve' button to complete your student's registration. 
*After approving the course request, you will receive an invoice and a link to select your payment options.

If your student is newly awarded a scholarship AND enrolled in/requesting a Florida Virtual School course, please check your student’s Florida Virtual School account to determine if we have received their scholarship information from the Florida Department of Education.
  • Scholarship information can be found by accessing the menu on the top left side of your Student Dashboard. Select ‘Student Records’ and then choose ‘Academic Profile’.
  • If you do NOT see your student’s scholarship information, please use the Self Reporting Tool provided in this form. Please use the questions below to determine if you need to Self Report your student's scholarship information.
*Failure to report your student as private pay may result in course access suspension and/or delays in future start dates.

*If you have declined the scholarship for the current school year, please notify us in writing at:  scholarshipinfo@flvs.net
