This summer camp is being organized by Brain Shine Academy (owners of Eye Level of Morris Plains) with expert coaching provided by Hadoop Express. The venue is Eye Level of Morris Plains, Powder Mill Plaza, Morris plains  NJ 07054. You may contact Hadoop Express for any enquiry regarding registration or camp activities. Please note that the camp is being held by Hadoop Express under the permit provided by Brain Shine Academy.

Cancellation policy is explained at the end of the form under terms and conditions. Additionally you will be required to conform to the summer camp policy provided by Brain Shine Academy including but not limited to code of conduct, medical record, pick-up and drop-off, etc.

Neither Brain Shine Academy nor Hadoop Express guarantees or provides warranty that the offered training will meet your requirements and/or class methods will be error free.   Student parents or guardians may not reproduce or transmit  material , methodologies or content provided at the camp for any reason without express permission of Hadoop Express.

Cancellation and refund policy:
It is assumed you will be fully paid before the start of your summer camp.  80%  of paid deposit is refundable in case of cancellation. 20% will be forfeited.
If the camp is partially attended before withdrawal, a pro-rated 20% of balance amount will be forfeited, Make-ups are permitted for any missed weeks or days during the camp..
PARENT AGREEMENT :                                
I am enrolling my above mentioned child (ren ) for the coaching provided by Hadoop Express for the robotics/coding camp to be conducted at 378 Route 46 W, parsippany New Jersey  (premises of Brain Shine Academy). I acknowledge that I have received and reviewed terms and conditions regarding the policy for admission and payment. I agree that as a parent/ guardian I shall hold myself responsible for providing further information regarding health record or any other information mandated by the State of New Jersey as required from parents and guardians registering for any youth camp.   I understand that  discipline and safe conduct is important for the running of the camp and shall abide by the policy regarding discipline and conduct as provided to me before the start of the camp.   In addition, I have read and agree to the following :
1. Camp organizers  shall not be responsible in case of sickness or injury of the enrolled child(ren)  while in attendance at the facility or in transit to or from the facility.
