We begin this program year offering only online programming due to safety considerations around the COVID19 pandemic. It may seem unnecessary to provide some of the information requested here. As our intention is to be together in person as soon as it is safe to do so, we are asking you to complete the full registration process to avoid the need to re-register in the future.

All children participating in the UUCC Children and Youth Religious Education (CYRE) Program or receiving childcare in the nursery must be registered. If you are unable to complete this online form, please contact CYRE staff at kathleen@uuccharlotte.org.

There are several pages in this online form. Please make certain that you read and complete all of the pages. You will receive a confirmation message when you reach the end. If you are uncertain whether your responses have been recorded, please contact CYRE staff at cyre@uuccharlotte.org.

The following information will be published in the Community Directory (members only): parents' names, addresses, home phone numbers, and e-mail addresses. For children, only their names will be published unless otherwise requested.
