This form will be active for our 2023 summer priority registration period, until October 31, 2022. If you are registering multiple groups to participate, please complete the form once for each group (ex: separate middle and high school youth groups looking to attend different sessions).

PLEASE READ THE FOLLOWING IN DETAIL:

1) The information collected in this form will be used to help create SSP's summer 2023 program schedule in early November. We cannot guarantee a specific program type will be held at any particular location or during a specific week, but will do our best to make sure all registered groups can be placed in one of their listed choices. You will be notified if we have any difficulty doing so, to ensure you are placed in a session that works for you.

2) The person completing this form should be the primary liaison for the group. By completing this form, you agree your participants will abide by SSP's Safe Community Policy, as well as SSP's safety requirements for COVID-19. SSP reserves the right to change our program details and participant requirements as necessary to abide by CDC and local safety regulations, and ensure a safe environment to protect our partner communities, volunteers, and staff.

3) At the end of this form, you will be asked to submit a flat group sign-up fee to register. Small groups (2-5 participants) will pay a $75 flat fee to register, and standard groups (6 or more participants) will pay a $200 flat fee to register. Your registration will not be complete until this is submitted.

4) By completing this form, you are agreeing on behalf of your group to be accountable for a deposit of $100 per person registered upon receiving your priority placement in early November, due by November 30, 2022 (credits may be used). You also confirm you have authorization to sign a contract on your group's behalf.
