Purpose

North Star Doula Service, LLC shares a goal with many doulas worldwide of promoting the status of doulas as part of basic standards of maternity care. One of the most powerful ways of showing what doulas do is through photographic imagery of the positive birthing experiences they foster. This release allows North Star Doula Service to use certain images and brief descriptions for website and marketing purposes. No imagery will be sold. Discretion will be used to preserve the privacy and modesty of the families they portray. If any question exists as to the appropriateness of the portrayal, every effort will be made to contact you for your input.

I grant permission for North Star Doula Service, LLC to publicly display, either in digital or print form, images and/or their descriptions from my birth with the following restrictions (please check one):

Unrestricted. Images and descriptions from my birth experience may be used without further consultation or disclosure

Conditional. Images and descriptions from my birth experience may be used if they are first submitted to me for my approval, and my express consent is given in reply.

Restricted. Images and descriptions from my birth experience may be used only if no personally identifying information is depicted or described (such as names, locations, or faces of myself, partner, or family members).

None. No permission is given to use any images or descriptions from my birth experiences except to edit, order, or print items at my own request and for my own use.
