Interboro SD COVID - 19 Notification for Close Contacts Outside of School
The quarantine guidance for close contacts that are NON - household members has changed.  

 For the team to evaluate all COVID -19 student cases and ensure that we contact parents that need additional guidance, we are asking parent to complete this form. A school nursing team member will only contact you if your information has been reviewed and does not align with quarantine/isolation protocols.

Close Contact Description  - A close contact is defined as someone who has been within 6 ft. of an individual who has tested positive for COVID- 19 for a cumulative total of 15 minutes or more in a 24 hour period.

Please complete the form in its entirety.  A separate form must be completed for each child within your household.
