6 Weeks:  June 5, June 12, June 19, July 18, July 24, July 31
High School: 7:00 - 8:30pm
Adult: 8:30 - 10:00pm

Registration Fee:  $80 (includes reversible jersey & official fees)
Mail payment in form of check or money order to PO Box 732, Mt. Laurel, NJ 08054.  Include player's name and team name on payment.  

Registration is due on or before May 26th.  Teams are first come, first serve.

In signing this waiver form below, I release Xplosive Sports Academy and other involved parties from any claims or responsibility for injuries suffered while participating in field hockey games. I knowingly assume all risks associated with participation, even if arising from negligence of the participants or others, and assume full responsibility for my participation. I certify that I am in good physical condition and can participate in sports. Further, I authorize the site director to request medical treatment as necessary to insure my safety and I will be financially responsible for such care. Also by signing below, I take all responsibility for knowing and abiding by the facility rules. Mouth guards and shin guards are required at all times while participating. I also give permission for Xplosive Sports Academy to use any videos and/or photographs of the participant for publicity, advertising or promotional purposes. *
