Individual members should use this form to report service hours. Any service hours over a month old will not be approved. Please do not report hours for more than one day/service event at a time.

Service hour reports will go straight to the offices of Fraternity and Sorority Life and the Center for Community Engagement. At the end of this form, members may choose to send a copy of this report to their own chapters.
