This form is for our registered cGaming volunteers to claim mileage with the following terms:

  • Mileage will be paid at the rate of: $0.68/km
  • Mileage will only be paid for your one-way trip to volunteer at the Rama Gaming Centre in Scarborough
  • We will also reimburse for public transit one-way to the Gaming Centre; please fill out information below and submit your receipts for reimbursement via email to info@opacc.org
  • Reimbursements will be done via e-transfer
  • Please fill out one form per date 
  • You must be a registered OPACC cGaming volunteer and confirmed to have volunteered on your assigned shift date
  • Total mileage for the month will be reimbursed all at once at the end of the month

Mileage claim:
1. Enter your starting address on Google Maps
2. Click "Directions" (blue arrow)
3. Choose your destination as 991 Kennedy Road in Scarborough
4. Select whatever route you took and multiply the number of kilometers given x 0.68
5. Enter the number below - this is the amount of your reimbursement which will be sent via e-transfer to the email indicated above
Public transportation claim: Enter amount of your public transportation cost(s) one-way to the Gaming Centre and submit receipt/email receipt to info@opacc.org
