It is expressly understood and agreed that the fees for all Sound Bath/Reiki sessions include no premiums for liability insurance. Instructors, their employees and/or agents shall not be liable nor shall any participant attempt to hold instructors, their employees or agents liable for injury or damage a participant or other persons or property either proximate or remote no matter how occasioned including without limitation injury or damage resulting from negligence of instructors or their employees or agent, and participant, hereby waive the right to collect any and all damages from instructors. Participant, shall indemnify and save harmless the instructors, their employees or agents from claims for damages for personal injury, including death or injury that may arise or may be alleged to arise in, on, or about the property, in any manner from the carryout of this contract, except those caused by the action or neglect or instructor. Failure be any participant to comply with any or all of the rules established by instructor, which failure to comply is the proximate cause of any death or injury shall not be deemed negligence of  instructors, their employees or agents. 

Be it known that in the event I can't be reached, I, the participant, do hereby give and grant unto any medical doctor or hospital my consent and authorization to render such aid, treatment or care to said participant as, in the judgment of said doctor or hospital may be required, on an emergency basis in the event said participant should be injured or stricken ill while attending an event at Suzy's Swim & Fitness Zone. I agree that the participant listed above is insured with private medical insurance. 

It is hereby understood that the consent/authorization hereby given & granted are continuing & intended by me to extend as long as the above named participant is registered at Suzy's Swim & Fitness Zone. It is further understood that any expenses incurred will be paid in full by the participant. Payment of the expenses in no way, even if promise expressed or implied has been made, a responsibility of Suzy's Swim & Fitness Zone. I agree to jointly and severally hereby releases, discharges, and acquits Suzy's Swim & Fitness Zone, including the instructors, relatives, employees and agents from any and all claims, damages, and injuries of any kind nature or description resulting from Suzy's Swim & Fitness Zone activities. This expressly includes, but is not limited to, any injury or damage caused by or resulting from the negligence of  Suzy's Swim & Fitness Zone, employees, teachers, agents, etc. I acknowledge the fact that certain types of  injuries are common and inherent in tumbling related activities. This release includes, but is not limited to this type of injury. This release shall be binding upon and inure to the benefit of the parties, their  successors, assigns, and personal representatives. By signing below, I agree to the above statements.
