If you are literally homeless, staying in an emergency shelter, camping or staying with friends and family without a lease DO NOT FILL OUT THIS FORM. Please go to the AHOPE Day Center at 19 N. Ann St and connect with our homeless services team. Due to an increased number of calls and requests for homeless prevention assistance, we are asking individuals at risk of losing housing to complete the following form. We will review your answers and if you are eligible will be in touch within 5-7 business days.
