I, the undersigned recognize and acknowledge that there are certain risks of physical injury to participants in the camp and I agree to assume the full risk of any injuries, including death, damages or loss, regardless of severity, which I or my child may sustain as a result of participating in any and all activities associated with this program. I, the undersigned parent or legal guardian of the child registered in the CanRef Youth Camp at Sunset Point, hereby release it’s organizers, directors, counselors, volunteers, and anyone else participating in the program, as well as Sunset Point Christian Camp, from any and all liability and waive any claims I may have against them for any loss or damage to the property of the participant or undersigned and for any personal injuries which the participant may sustain as a result of or arising from participation in the camp program. I authorize any of the adult supervisors to administer over-the-counter meds such as Tylenol, Advil, cold or allergy medicine, etc. as needed (Please inform us if you are packing any meds for your child). I authorize any of the adult supervisors to consent to any medical attention that my child may require and to make any arrangements they feel are required for special transportation (e.g. by ambulance). I assume all responsibility for the cost of such special transportation. I also understand that should my child disobey the camp rules I may be contacted to come and pick him/her up from the camp. *
