Please complete this application to begin the enrollment process for your child in the Heber Virtual Academy an Independent Study Program in the Heber Elementary School District. The Heber Virtual Academy will be nearly identical to our current Distance Learning Program.  Students applying to the Heber Virtual Academy will be evaluated to determine if the Distance Learning Program is an appropriate option.  Please complete both the Distance Learning Application and the School Enrollment Forms to reserve a spot for your child.   SPACE IS LIMITED.
