This form is for parents and students who have been approved for Virtual Shelby.

Please check each of the boxes below to indicate that you have read, understand, and agree to ALL of the following guidelines: *

I have visited the Virtual Shelby website and read the Virtual Shelby Orientation and Handbook.

I understand that my student must meet and maintain the admission requirements listed on page 4 or 5.

I understand that my student is making a full year commitment to Virtual Shelby.

I understand that my student will have to participate in mandatory state assessments in-person at the school in which they are enrolled.

I understand that my student must maintain adequate progress (pg.8) by keeping passing grades, staying on pace with the target completion, and working actively each week in each of their courses.

I understand that my student will be required to return to the traditional classroom setting should they fail to make adequate progress or fall below the required admission requirements.

I understand that my student will not be allowed to come on campus during the school day to participate in any extracurricular activities.
