If you do not fill out this form prior to 9pm on Saturday 10th, we cannot guarantee you a place at a taster session.

All players must undergo a self-assessment for any COVID-19 symptoms. No-one should leave home to participate in sport if they, or someone they live with, has symptoms of COVID -19 currently recognised as any of the following:
-- A HIGH TEMPERATURE
-- A NEW, CONTINUOUS COUGH
-- A LOSS OF, OR CHANGE TO, THEIR SENSE OF SMELL OR TASTE

Should an individual have demonstrated any such symptoms, they must follow NHS and PHE guidance on self-isolation: https://www.nhs.uk/conditions/coronavirus-covid-19/symptoms/

You must NOT attend if you have been instructed by contact tracing authorities (NHS Test and Trace / Test and Protect / Test, Trace, Protect) in your location to self isolate for a period covering the date of this training session.
