Due to the global COVID-19 pandemic, the Maywood Board of Education has decided that all students must wear masks or other face coverings when dictated by the School Opening Plan or circumstance dictate the wearing of masks.  This also includes participating in school activities, with certain activity-based exceptions, unless their parents objects to their student being required to wear a mask or other face covering.

If you have no objection to your student wearing a mask while at school and related activities, you do not need to do anything.

If you object to your student wearing a mask in order to reduce the risk of your student transmitting or contracting COVID-19, you must sign and return this opt-out form no later than September 1, 2023.  If you object to your son/daughter wearing a mask, you also understand that should an outbreak of COVID-19 occur they may need to stay at home until the outbreak subsides.

I do NOT wish my student to wear a mask while at school.
