Dear friends, to be able to properly organise the COVID-19 testing for those who need it, we ask you to fill this ANONYMOUS form so we can organise the testing in an efficient manner.

IMPORTANT!
-> Please YOU DO NOT NEED TO FILL THIS FORM if you have been vaccinated in the last 270 days or had a corona in the last 6 months and you have an EU COVID-19 passport.

-> Self-bought tests are not enough. We can only accept tests done by official medical staff/lab which can issue a QR code (EU Covid-19 passport) that we can check and read with official CovidGO application (available for Android OS and iOS).
