If you have more than 1 child participating, please use separate sections for each child. If you are in Trees & Saplings, also use separate sections for each participant including the adult(s) attending. Please fill in Name & Pronouns, birthdate, and day and group (ie Tues Wild Explorers) for each participant.

There is space to put your Ancestry, this is to acknowledge/honor the people you come from, not to classify groups of people. If this doesnt feel right to you, please just pass. Is there anything that will be helpful for us to know about you or your children.....behaviorally, allergies (we will have more on this on the med form, but its nice to know before the first day), gifts, passions, challenges, needs?
Covid: We plan to start the year with no masks and ask that each child has a mask with them in their backpack. Kids are welcome to wear a mask if they like.  We will be revisiting our illness policy and will send out the updates before we start groups. As with everything in life, we will shift and adapt as things around us change. We will be in communication re any changes. We will reach out to the group with particular scenarios. An example: child A had Covid, symptoms started 8 days ago, but still has runny nose, are you all ok with them attending today?

Does all of this work for you? Do you have any health considerations that make you or someone in your home vulnerable to complications related to getting Covid?

*Although we have all of our groups at Lake Farm, we may schedule some days at other places, like field trips. Some of those are known up front, like visiting Donald Park for our Maple Syrup day. Some of those come as opportunities throughout the year. It is rare, but please note that opportunities might arise where it makes sense to visit a different place one day. We will support with carpooling and logistics if and when that happens. We operate on seasonal rhythms, weather patterns, passions, curiosities, and opportunities.

MTtS - We will announce our location in an email. It is likely we will end up on private property in rural Barneveld at times, possibly Donald Park at times, and possibly somewhere else.

Communication! is key! We value and thrive when there is good and clear communication as much as possible. Please reach out  your mentors and/or Heather and Alex with any concerns from you or your child, even if they seem small. The sooner we can work together to resolve things, typically the better the outcome. This could be things like ongoing conflict between two kids, concern around covid, your child complaining of the cold and being unhappy at the end of the day for a couple weeks in a row, etc. Let's work together!

Please give any feedback you might have re this form
Do you have any interest in organizing community type things...like each group having a tea time for parents in the morning once a month, or a monthly or seasonal potluck/picnics for all of whnc or particular your group, etc? Or other ideas like that.
*
Is there anything else you would like us (Heather and/or the mentors/grandparents) to know about you, your family, your child, or anything else?
