Parent Liability and Responsibility Agreement
1. I will assume full responsibility for the safety of my child(ren) while participating in Kid A Workshop.
2. I will not hold the Kid A Workshop liable for any personal injury or property damage, which may occur on the premise of the school.
3. I will assume full responsibility for any damage caused by my child(ren) to the facility used by Kid A Workshop.
4. Paying period is every month or one semester. Tuition is non-refundable (Exceptions for special circumstances may request in person).
5. Registration fee are non-refundable.
肖像权使用授权书 Permission to Use Photograph
I hereby grant the permission to Kid A Workshop, to take and use: Photographs and/or digital images of my child for use in news releases and/or educational materials as follows: Printed publication or materials, electronic publication, or Web sites. I agree that my child's name and identity: may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. All negatives, prints, digital reproductions and shall be the property of Kid A Workshop.
