The Rhode Island Department of Health (DOH) recommends that all employers put COVID-19 screening protocols in place to help prevent the spread of COVID-19 in their facilities by screening employees and visitors on a daily basis. The screening protocol that follows has been developed to align with the DOH recommendations. This form must be completed daily  before entering the building.
