Fill out this form to create an appointment to get a 24 hour membership. THIS FORM MUST BE FILLED OUT 3 HRS PRIOR TO APPT TIME TO ENSURE STAFF CAN MEET YOU. Once you submit your appointment is CONFIRMED. If anything changes for you please email myuptowngym@gmail.com asap to change your appointment time or cancel. Sign up takes approximately 15 minutes. Please bring your phone with you. The door at the gym will be locked when you arrive, a staff member will see you at the door and let you in. Expect to pay for your membership online myuptowngym.com and be sure to select the $20 activation fee OR by etransfer while you are at the gym for your appointment.
