Brookfield Soccer - Field Usage Request Form
Please use this form to request field usage for Brookfield Soccer Fields. Requests should be submitted 2 weeks prior to the date they are needed to allow the BSC Board to review the request and to vote on it. Requests should not be considered approved until you have been notified by the BSC Board, and should not be publicized until they have been approved. 1. Team description – include club name, team age, team gender
