Special Olympics Athlete Enrollment 2023-2024
 Dear Parent(s)/Guardian(s),

Please complete the information below if you wish for your student to participate in any one of the Special Olympics sports or programs offered by the Grand Prairie ISD delegation listed below during the 2023-2024 school year. All approved off-campus activities of athletes, coaches/chaperones, and peer partners will travel with Grand Prairie ISD on transportation provided by the District. During these events, Grand Prairie ISD Special Olympics will be under the auspices of Special Olympics Texas, as well as Grand Prairie ISD. Athletes will abide by the guidelines and regulations established by both organizations. In addition, policies established by Venue sites will be enforced.

By completing the information below, you are granting permission for your student (must be 8 years or older) to attend and participate as an athlete in one or more selected Special Olympics sports and activities throughout the school year pending administrative approval. In order to participate in after school team sports, athletes must be in grades 6th or higher (secondary). You, further understand that while student safety is a high priority for the District, under state law, the school is not responsible for medical costs associated with a student injury. 

By completing this enrollment, you acknowledge that it is your responsibility for filling out the correct information and enrolling your athlete in the appropriate sports. Any mistakes made on your behave, must be communicated to the Districts Special Olympics Coordinator within a timely manner in order for your athlete to participate. Failure to communicate within a timely manner will result in your athlete being unable to participate in that specific sport.   

You, expressly waive all claims for medical expenses, loss of services, or other claims, and agree to indemnify and hold harmless the District, its Trustees, employees, and agents from all claims made against it or them on behalf of your student.

You, agree to indemnify and hold harmless the District, its Trustees, employees, and agents from all claims made by third parties against it or them which results from your student's actions on the trip.

You understand that the District, its Trustees, employees, and agents are not waiving any sovereign or governmental immunity which it or they have under Texas law.

You acknowledge that these events are public and athletes, coaches/chaperones, and peer partners will potentially be photographed and/or be video recorded by the District, event facilities, and/or other media outlets.

You have read and understood this release and sign it voluntarily and with full knowledge of its significance.

This release applies to all approved trip(s) during the school year.

Note: The sponsors of any student trip should take with them copies of each student's Authorization to Secure Emergency Medical Treatment. Copies of this authorization may be presented to the admissions office at a hospital, clinic, physician, or dentist. Other distribution shall be only with the limitations of the Family Education Rights and Privacy Act.
