Weekly Work from Home Report
At the end of each work week (before midnight on Sunday) please fill out the following form to report any time you spent working on library tasks from home. Forms received past the Sunday 11:59pm deadline may be held for the following week.

Please note that you are not required to work at home and are under no obligation to fill out this form if you do not wish to report work done remotely. Also, there may be weeks when you do not have any offsite work to report, particularly if you are working onsite at or close to the number of hours you were working prior to the pandemic.
