Please use this form to book your JUNIOR SCHOOL childcare provision for both Before and After School Clubs for the 2nd-27th November only.

For After School Club, please ensure to indicate your intended collection time on each day your child will attend.

If you are unsure of your requirements for the whole period, please just complete the information that you currently require and then resubmit a further form at a later date.

Please note that we can only accept forms submitted by 2.30pm on the day you require your child to attend.  In an emergency for any late requests after this time, please ring the School Office on 02380 261808.
