PTO Grant Request Form
Grants may be used to improve the intellectual, creative, social, emotional, and/or  physical well-being of White Township’s students.
Please note:
1. If you can attend the next PTO meeting, please join us to tell us about your request and answer questions. (This is NOT required, but can be helpful!)
2. If the purchase is dependent on PTO funding, please wait to be notified of approval or denial before ordering.
3. All items become the property of White Twp School and must remain in the building, unless otherwise specified.

What is the cost of this grant? Has "price shopping" been done for other vendors, if applicable?
Do you request reimbursement to yourself, or that payment be made directly to a vendor? Receipts or invoices are required before payment. They can be put in the "PTO" box in the office.
When do you need an answer by? Is there is a due date for payment?
