This is the official entry form for the 2022 Stories on Stage: Adapting Short Stories for Audio Drama Competition. Please complete all sections relevant to your entry. In Section 2, the Entry Submission Area, it is important that you paste the link to your Google folder that contains the following documents. The documents need to be named as shown below.
■ Entry Form (School name_Entry Form);
■ Pledge Form (School name_Pledge Form);
■ Script (School name_Story Title_Script); and
■ The audio recording (School name_Story title).

Section 3 requires the names of the students involved in making the audio drama. Please write them individually in the specific order as shown in the relevant box.
