NFBTX Mentee Application Form
We accept applicants who are ages 14-22. If you are already 22 or older, you are ineligible for the program. If you are not yet 18, please use the application for minors. You can find that application on our website. Or, you can call our office and request that we send you the link to that form. If you have difficulty completing this form, we can help. Just call our mentoring program at 281-968-7684.

Eligibility Requirements are as follows:

Must be aged 14-22. If you are 22 or older at the time of application, you are ineligible to participate in this
        particular program. Feel free to contact us to find out if we have other programs that might fit you.
Must be participating in an educational program. This includes middle or high school, a post-secondary
        educational institution, or an adult rehabilitation program if your plan is to transition to a college or
        university following training. Unfortunately, we cannot accept individuals who are not pursuing an
        educational program.
Must have an open case with the Texas Workforce Commission's Vocational Rehabilitation Services.
Must be legally blind. The definition of legal blindness in the United States is, a visual acuity of 20/200 in
        your better eye with any correction (glasses, contacts) you may use to see better, or a field of vision no
        greater than 20 degrees in the better eye. So blindness in one eye is not considered legal blindness.
