SHARP COVID-19 & Racial Justice Community Survey
Dear Survivor Community

We invite you to participate in our SHARP COVID-19 & Racial Justice Community Survey.  We created this survey to ensure that government services are accountable and responsive to the needs of survivors of sexual harm. SHARP is eager to hear your thoughts on our COVID-19 and racial justice response and action, thank you for taking the time to share your thoughts.

The survey is divided into three sections.  All sections are voluntary, please fill out what you feel comfortable sharing with us.

- Section One is your name and contact information.
- Section Two is tailored to members of the survivor community (including family members, friends, etc.)
- Section Three is directed towards service providers
The primary goal of all questions is to invite all participants to share their knowledge and feedback with SHARP.

You will be able to navigate through sections using the “Next” and “Back” options at the bottom of the page.

The goals of this survey are to:

1. Identify survivor needs
2. Address existing gaps in services
3. Inform community on opportunities to support efforts during the COVID-19 pandemic
4. Inform community on opportunities to support efforts during the Black Lives Matter movement

Please contact SHARP with any questions at sharp@sfgov.org. Please feel free to share this survey with other members of the survivor community to ensure that all voices are heard. You may also contact SHARP by phone if this survey is not accessible online.

Thank you for your support and for sharing your voice!
In solidarity,
