East High School Scholarship Application

The Madison East High School Scholarship Committee requests that you answer all of the following questions as completely as possible, so that you can receive full consideration for all available scholarships. The deadline for the online application is January 20, 2023.

You are able to return to this application at any time to make edits up until the deadline date. To do this, you must "SUBMIT" every time you make a change, even if your application is only partially complete. You will need to put something in each box. We suggest putting "Not Done" in boxes you wish to come back to. An email will be sent to you with instruction on how to re-enter your application to make changes.

EHS 2022-23 Scholarship Application

The East High School Scholarship Application Eligibility Requirement Includes ALL of the following. You must be able to check ALL of the boxes to qualify. *

**A STUDENT WHO HAS BEEN ATTENDING EAST FOR A MINIMUM OF 3 FULL SEMESTERS**

A senior that is on-track to graduate

Must be committed to participating in the Senior Awards Ceremony (provided it's safe enough to hold an in-person event) OR it may include photo submission for virtual recognition or follow up with student bio information (i.e. post high school plans)

If there are any changes to this application after you submit, you will inform Ms. Murphy rlmurphy@madison.k12.wi.us

I understand that scholarships can be withdrawn if it is determined that information submitted on the application is inaccurate. (includes failure to respond to requests for updated information)
