EVERY MEAL PERMISSION (Formerly Sheridan Story)
If your family is in need of a little extra food over the weekend, please consider participating in the Sheridan Story. At the end of every week, school staff will put a small bag of non-perishable food in your child's locker for the weekend. Staff will only be given locker numbers and not the identity of families participating. Students should be able to put food into his/her backpack to carry home easily and discreetly. Each bag of food contains fruit and vegetables and provides nutritious, substantive meals for kids to eat at home. A sample bag from our inventory includes the following items: a can of pears, a bag of rice, a can of green beans, a can of tuna, a bag of pasta, and a can of chicken noodle soup. Only school administration will see the names in order to run locker numbers. If you have any questions, please contact your child's principal, guidance counselor or social worker. THE FIRST DISTRIBUTION WILL AT THE END OF SEPTEMBER. If you have more than one child in the program, please consider signing up each child.
By filling out this two question form, you are giving permission to provide this service. You are also agreeing to the following terms:
- I understand that if my child has allergies, it is my responsibility to remove any food items that my child should not consume.
- I understand that my child is responsible for keeping the food inside the bag in which it is provided until he/she arrives home.
- I understand that if at any time my child cannot responsibly transport the bag of food home, I will have to pick up the food at the school or no longer participate in the program.
- Your child's locker will be opened for the purpose of putting food in the locker.
- I have read and agree to the above statements and signify the same by entering my name and completing the form below.
- If at any time you would like to discontinue this service, please contact your child's guidance counselor, social worker or principal. Thank you!
