Did you receive your sacraments at St. Vincent de Paul Catholic Church and need a copy of a sacrament certificate? We can help! All Sacramental Records must have a written request. Please fill out the form below and your Sacramental Record request will be fulfilled within 2 business days.

*Note: Only the person who received the sacrament or their parent/guardian may request sacrament certificates
