Once this has been completed you will have access to the Colorado River BOCES Equipment and Assessments.

In the interests of maintaining a more accurate inventory of property which Colorado River BOCES loans to district and Colorado River BOCES employees for their use with students, Colorado River BOCES has prepared individual and district agreements to clarify expectations, conditions, and responsibilities. Both Agreements must be updated annually. Districts are also responsible for communicating clearly to parents that all materials are on loan and need to be returned at the end of the loan period.

Materials available for short-term (weeks) and long-term (up to a full school year) use include but are not limited to:
• Instruction and evaluation tools
• Assistive technology, audiological and other equipment for occupational therapy, school
psychology, speech language pathology, vision, deaf/hard of hearing
• Resources (DVDs, CDs, videos, books) regarding professional development, alternative licensure, specialized topics such as autism spectrum disorders

Signed EREC District and Individual Agreements must be on file at Colorado River BOCES prior to allowing district employees to check out EREC items. For assistance regarding any part of this process, contact EREC Coordinator, Kristy Slife at kslife@crboces.org.

EREC Member Agreement

By completing the information in the EREC Agreement, I acknowledge that I understand the expectations, conditions, and responsibilities set forth in the Educational Resource and Equipment Center (EREC) Individual Agreement.

I understand that I will be registered as an EREC member and will retain loan privileges for one year, as long as I remain employed by the Colorado River BOCES or member district and I am in good standing with the Educational Resource and Equipment Center.

Checkout and Return Process

By checking each box, I acknowledge that I understand the expectations, conditions, and responsibilities set forth in the Educational Resource and Equipment Center (EREC) Individual Agreement.

Checkout and Return *

Checkout for EREC items should come from the staff member who will be using the item and taking direct responsibility for the item’s safekeeping. All requests will be honored on a first come, first served basis.

Loan time restrictions vary. Most items may be borrowed for up to six weeks, unless another employee requests an item sooner. Loan time may be extended if no one is waiting for the item. Assistive technology resources are intended for evaluation and trial purposes only.

Software may be installed on ONE computer at a time during the loan period, and it must be uninstalled prior to returning it to Colorado River BOCES. The borrower is liable for license violations if the software is not uninstalled.

If an EREC item arrives damaged or inoperable or has parts missing from the content list, the borrower is responsible for notifying the EREC Coordinator immediately. If repair work is required, the item must be returned to Colorado River BOCES.

EREC items MUST be returned in clean, good working order. It is imperative that the responsible individual confirms prior to shipping that all parts (i.e., adapters, power strips, manuals, toys, protocols, etc.) are present and in good working order and are in the original shipping container with all packing materials (refer to the contents list included with the item).

Any item returned to Colorado River BOCES by U.S. mail MUST be insured for the replacement value. We encourage borrowers to use the Colorado River BOCES courier system, which is free of charge.

Lost, Damaged, or Broken Items

L/D/B Item *

Should an item be broken or otherwise damaged by a student or anyone else during a loan period due to anything other than normal wear and tear, replacement/repair costs become the financial responsibility of the borrower and the school district. Note that highlighting, underlining, and torn or bent pages in books or manuals are NOT considered normal wear and tear and are not acceptable.

Replacement of lost or incomplete EREC items is the financial responsibility of the borrower and the school district.

Any employee with overdue items or a pending bill for lost or broken items will not be allowed to borrow additional materials until the overdue items are returned and/or the bill is paid in full. In addition, failure to return items or settle accounts could cause the suspension of access to EREC for the rest of the staff in that district.

District Special Education Administrators will be notified of overdue, lost, and damaged items.

Borrowers DO NOT have permission to loan, trade, sell, or upgrade any EREC items. You are welcome to contact the EREC Coordinator if you learn of such opportunities.

Assistive Technology Equipment

Assistive Tech *

Requests for assistive technology devices should be accompanied by a typed assistive technology implementation plan.

Assistive technology items may be checked out for as many as six (6) weeks. Loan time may be extended if no one is waiting for the item.

All required components should be included in one request. For example, include a communication device and a switch access kit, if the student is not able to access the equipment directly.

Assistive technology resources are intended for evaluation and trial purposes only. If a student requires an assistive technology item for long-term use, it will be up to the district to determine how best to procure the same type of item for continued use by the student.
