Although the safety of all sport activities is the primary concern, indoor sport activities with The Basketball Experience may cause injuries and/or death. I expressly assume the risk of injury, death, disability and/or illness arising from any cause, and agree to waive the right to pursue any claim against The Basketball Experience and the person(s) in charge. This is to certify that I, as parent/guardian with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of the activity and his/her responsibilities for adhering to the rules and regulations. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s/ward’s involvement or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.   *

I, the undersigned, do hereby grant permission to The Basketball Experience to take my and/or my child's photo taken during trials, practices or any other events hosted by The Basketball Experience and use them on their website, social media or any other advertisement media. I hereby release The Basketball Experience, their representative, employees, managers, members, from all claims and demands arising out of or in connection with any use of said photos, including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights. I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and/or exploitation of the photos or any rights therein. I also acknowledge that my child is under 18 years old and lack the legal capacity to enter into binding agreements. Accordingly, I have read this Release and consent to my child's inclusion in the photos will not contest the rights granted in this Release, and shall assist and support you in any and all legal proceeding for affirmation of this Agreement, should you choose to have a court of law affirm this Agreement *
