The DanceWorks Kenora Health Check must be completed each day that the dancer is attending a class at the studio. Families are responsible for ensuring that the answers have not changed between completing the screening and attending the studio.

Parents/guardians can fill this out on behalf of a child. *If you have more than one dancer attending on a specific day please complete one survey per dancer.*
