Golden Strip CC Driver/Rider Permission Form
This form should be completed for any student that wishes to drive themselves to and from the career center, any student that wishes to ride with a student driver, or any student that is dropped off/picked up by a parent/guardian. (NO fee for parent pickup)

This form must be fully completed and permission granted BEFORE the student drives, transports, or rides to or from Golden Strip Career Center. A $10 fee is required for drivers or riders after the application is submitted.
