Thank you for donating masks! This form is for logistical purposes. So we know how many to prepare for. These masks will be greatly appreciated as it will ease the financial burden of those who may need extra masks.

Guidelines for masks:
- Reusable
- New/not used
- No Political or profanity wording or logos.
- Must be put a sealed bag (does not have to be individually packaged)
